Case Study: How Sage Detect Transformed Fall Management at CountryHouse Omaha

Leaders at Agemark Senior Living wanted to improve resident safety, specifically around falls. Not only are falls the leading cause of injury for those aged 65 and up, but they cost enterprise communities an average of $712,000 per year. Moreover, leadership knew that traditional fall detection systems weren’t enough—too many falls went unwitnessed, response times lagged, and follow-up care often lacked the context teams needed to prevent recurrence.

In early 2025, Agemark’s CountryHouse Omaha became one of the first communities to pilot Sage Detect, an AI-powered fall management system which is fully integrated into the Sage Core nurse call platform. Within just five months, the community saw transformative results: faster responses, fewer falls, and much stronger resident outcomes.

The Challenge

CountryHouse Omaha faced several key challenges in fall management. The team had limited visibility into incidents. Without real-time insights, caregivers could not respond to falls as quickly as they wanted and didn’t always understand why a fall occurred.

The community also had a reactive response model. Without real-time alerts, residents spent more time on the ground after falls, which raised the risk of complications like dehydration, pressure injuries, and hospitalization.

Finally, their fragmented workflows meant that fall detection and management fell outside of their nurse call system. Caregivers had to juggle multiple tools and processes, creating confusion, slowing communication, and delaying response times.  

The Solution

Integrated directly into the Sage Core nurse call workflow, Sage Detect uses AI-powered camera sensors to identify falls in real time and alert caregivers. Through the Sage app (which caregivers already used to document daily care), they could now receive real-time fall alerts with resident details and location. After falls, staff with pre-approved access could review footage of the incidents to understand contributing factors to the fall. Operators also had access to the Sage Fall Dashboard, which surfaces trends about fall timing, frequency, response, and recurrence. This helped them focus on follow up steps which could deliver the greatest safety and operational impact.

Even with minimal tech experience, CountryHouse caregivers quickly adopted the system. The Sage team provided full support—from installation and onboarding to real-time coaching—ensuring a seamless transition and immediate results. Because Sage Detect works using cameras installed in residents’ rooms rather than a pendant or another device, residents had no learning curve for use and were automatically monitored once the system was turned on.

The Results

Within five months, CountryHouse Omaha saw dramatic, transformative outcomes across every metric, including:

  • 55% faster response times, cutting down their average response time from nearly 15 minutes to just 6.6 minutes.
  • 50% fewer falls, as proactive adjustments and real-time visibility helped caregivers prevent repeat incidents.
  • 75% fewer fall-related hospitalizations due to faster response times and the ability to review footage.

With Sage, caregivers only needed a single platform for both nurse call and fall detection, which drove fast adoption and more coordinated responses. And, for the first time, clinical leaders had objective data and visual context for every incident, which allowed for them to make confident, evidence-based decisions around care and escalation. Sage Detect’s in-app replay paired with the Sage Fall Dashboard analytics platform allowed the team to review each incident, identify contributing factors, and make targeted improvements, such as care plan updates, staff retraining, or room adjustments.

For example, caregivers noticed that, after dinner, one particular resident was placed in her wheelchair with the pedals up, which made her restless and eager to walk—and made her more at risk for a fall.

“After dinner, our team now helps her transfer to a comfortable recliner where she can relax and unwind with one of her favorite shows. Since making this small, personalized change, she hasn’t had another fall,” said Trisha Riddle, executive director at CountryHouse Omaha. “This kind of insight is possible thanks to the video technology that helps us better understand and support each resident.”

Following the success at CountryHouse Omaha, Agemark decided to expand Sage Detect to 20 additional communities nationwide. This solution has not only helped improve the confidence of caregiving teams, but has created more trust between Agemark and the residents and families it serves.
