Sage unites the data that matters most—integrating with leading EHRs to give operators and caregivers one reliable view of care. Today, Sage is fully integrated with ALIS, ECP, and Yardi, with PointClickCare launching by year end and an integration with August Health in early 2026. Together, these partnerships represent coverage across more than 75% of the senior living market, giving operators flexibility and choice while keeping data fully synchronized across systems.
Each EHR integration enables the same core capabilities: streamlined onboarding, real-time resident profile sync, and automatic level-of-care updates—reducing manual entry and improving accuracy across every record.
By automatically syncing resident profiles, updates, and care documentation across systems, Sage significantly reduces duplicate entry, minimizes manual effort, and ensures that everyone—from frontline caregivers to regional leaders—has a consistent, real-time view of care.
Communities using these integrations are already going live faster, onboarding residents more efficiently, and spending less time reconciling data across platforms. As Sage deepens these connections, we’re building toward true interoperability—where insights, actions, and documentation flow seamlessly between systems to keep residents safer and teams more connected.
As these partnerships deepen, Sage is expanding its functionality to support richer data exchange—including alerts, tasks, and other care-critical signals—enabling real-time insight sharing across systems.
Now live. The integration updates resident profiles, ADT information, and level-of-care within minutes—reducing manual entry and supporting accuracy across every record.
ALIS powers communities nationwide with advanced care management and compliance tools; together, Sage and ALIS make it easier to maintain a single, reliable source of truth.
“Together with Sage, we’re giving operators a more complete and connected view of care,” said John Shafaee, Co-Founder and CEO of ALIS. “This integration helps communities reduce manual work and maintain real-time accuracy across systems—strengthening both compliance and quality of care.”

Now live. Trusted by more than 8,000 senior living communities, ECP’s integration with Sage securely syncs resident data in real time. This connection helps close the gap between frontline care activity and clinical documentation—giving staff the ability to document more accurately and maintain a consistent view of resident information across systems.
“Our partnership with Sage is focused on making caregivers’ work simpler and more connected,” said James Baumeister, Product Manager at ECP. “By linking ECP’s clinical platform with Sage, operators gain immediate visibility into what is happening on the floor.”
Now live. Used by more than 3,000 senior living providers, Yardi’s integration with Sage automatically syncs resident information and ADT data at regular intervals—keeping records current, reducing manual steps, and ensuring smooth day-to-day operations.
Fully contracted and launching before the end of the year. PCC is one of the most widely used EHRs in post-acute and senior care. Sage’s integration will automatically sync resident updates and transfers, ensuring care teams always have the latest information—right where they work.
In testing. Sage and August Health will launch their integration in early 2026, expanding connectivity and giving operators even greater choice in how they unify data across systems.
“Senior living teams deserve technology that gets out of the way so they can focus on residents,” said Erez Cohen, Co-founder and Co-CEO of August Health. “Integrating with Sage gives caregivers a clearer, more connected view of every resident, with real-time data that strengthens clinical decisions and improves outcomes. Together, we’re pushing the industry toward true interoperability and more empowered caregiving.”
Sage’s integrations are setting a new standard for connectivity in senior living—bridging the gap between care, data, and decision-making. As we continue expanding our partner network and capabilities, we’re bringing the industry closer to true interoperability.
